Merge the php 7.3 branch with the Pantheon APM_DRIVER_HTTP changes.

I didn't mean to open this branch here. A brief explanation:

This PR adds an HTTP driver that allows APM to report to a remote monitoring system. This driver has been in use at Pantheon for many years. It might benefit others if merged here.

There are test errors here at the moment, so I am going to close for now.
