Get [online] VSIX installer working #1

The offline msi setup file seems to be working fine for now on VS2008 & VS2010.
VS2010 added a new VSIX install option that allows online installation.
The basics of this is in the checked in code, but it needs a few tweaks to get 
working correctly.
