Change title in Uploader to something other than Nightscout #272

The uploader software has a large header on top labeled as "Nightscout", which is leading to confused support requests where users have trouble describing which component in their Nightscout setup is potentially having issues and/or are having issues following instructions on fixing issues due to thinking Nightscout is running on their phone. Ideally the uploader would use very explicit language on what part of the system is the uploader and what part is Nightscout (the backend).

Ping! I'm again advising a user who's super confused on what Nightscout is due to this title + she's assuming the phone app is buggy due to not showing the full Nightscout view.

FYI this single issues has caused an amazingly huge amount of hassle during the last month with people migrating to Atlas and being confused around being able to describe where they're observing issue. Please please please fix this asap
