Summarized Alarm Status Widget #134

Collect a number of signals summarizing the highest level alarm status in a single indicator. This should be usable in QtDesigner as well.

This is contingent on https://github.com/NSLS-II/ophyd/pull/653 which does not look like it will make it an ophyd release in time for the v0.3.0. Delaying until next release...
