Wire up order submission workflow

1. Click submit order
2. pop up modal to collect user name, email, phone
3. user clicks submit - spinner while data posted to /order endpoint
4. message for user to "check email for order verification email"
5. also provide a link to "resend order verification" which will take them back to the form to update name/email/phone in case they messed it up
