Allow for seperate cover art for each disk on a title #78

Each disk on a title should allow for its own cover art, this cover art 
should override the default title cover art when in the details screen.  
So as a user switches between the play buttons for each disk the cover art 
should change to the cover art for that specific disk if it exists.
