File upload failure in Alfresco 5.x #6

What steps will reproduce the problem?
1. Install customise-site-logo-share-42-2.0-SNAPSHOT into 
tomcat/webapps/share/WEB-INF/lib
2. Select "Change Site Logo" in site gear menu
3. Upload image

What is the expected output? What do you see instead?
Get error in file upload dialog "3 KB (Failure: 404 Not Found)"

What version of the product are you using? On what operating system?
Alfresco One/Enterprise v5.0.0 on Windows 7 Pro

I have replicated the bug in Alfresco 5.x Community in the same way.
Installation and activation of the addon seems to work fine.
The 'Change Site Logo' menu item appears correctly, and selecting it brings up 
the Upload New Image page.

The upload fails every time, regardless of which image is uploaded.

The dialog title: "Uploading file to {0}" seems wrong - should it display a 
location?
