Update to 9.x compatibility #14

Hi,

I needed a migration package and saw that none was updated to work with 0.9+ yet (at least none that I could find).

So I gave this a quick overhaul. I can't guarantee nothing, and the command line script definitely won't work as described I think.

Please have a look and I'm looking forward to feedback as well.

Thanks @DanielDornhardt . Unfortunately you're a couple hours late to the party as I pushed an updated version (working with 0.9) to atmosphere this morning. Thanks for contributing though!

Alright, good to see an official version.

But actually you might want to check/update the docs again, or maybe copy a sentence or two from my changes: With the new Meteor package management, I don't see the packages inside the packages/ - dir anymore when they are installed via "meteor add". So the command line scripts won't be there in a regular install I think.
