remove InCommon references from document "simplesamlphp-ukaccess" #608

InCommon made some changes to its infrastructure recently, which caused some of 
the content on documentation page "simplesamlphp-ukaccess" to become outdated. 
Rather than try to fix it, I recommend all the InCommon references be deleted, 
except for a reference to the following page.

Some new documentation about metarefresh has recently appeared in the Internet2 
Spaces wiki:

https://spaces.internet2.edu/x/eYHFAg

For the time being, if you would link to the above page, that would be helpful.

Here's a general comment and suggestion. Documentation regarding metarefresh is 
spread across two pages:

http://simplesamlphp.org/docs/stable/simplesamlphp-ukaccess
http://simplesamlphp.org/docs/stable/simplesamlphp-automated_metadata

This has always been confusing to me. Personally, I would move all the 
metarefresh documentation into the latter page, but in the very least these two 
pages need to clearly link back and forth to each other. In particular, the 
latter needs to refer to the former, but you can minimize the possibility of 
introducing a documentation error down the road by consolidating all of the 
metarefresh documentation into the latter page.

Hi Tom,

I've removed the ukaccess page, moving all the info that was in no other pages 
to where it originally corresponds. That means improving (heavily) the 
automated_metadata page, but also the basic sp documentation.

Everything is available in trunk now.
