Remove broken PlanToEndEffectorOffset. #14

The PlanToEndEffectorOffset has never worked properly and confuses people, so I am removing it.

I have created a feature/PlanToEndEffectorOffset branch with the latest version of the code from master, if we want to use that as a base for a proper implementation.
