Port d7 commits to master #239

This PR ports several D7 enhancements to the master branch. As far as I can tell, my changes will bring master current with D7 with the exception of https://github.com/perusio/drupal-with-nginx/commit/3009c0e80bca953391e0f1fcf42ce70cff1c6d34 (see https://github.com/perusio/drupal-with-nginx/pull/218).
