Refresh API doc index #616

Was missing ena_submit, and wasn't sorted.

Trying to include __init__ and __main__ is non-trivial, unclear why. Probably a side effect of who I configured this originally - might be worth revisiting that as an automatic index would be better.

How strange, tests/test_ncbi-import.sh is failing

Test failure due to yesterday's release of xopen 2.0.0 which breaks cutadapt: https://github.com/marcelm/cutadapt/issues/774
