create templates for repository #39

Would like PR template and issue template, possibly need to add additional templates.

Should look into recommended basics.

Added issue template for documentation request in #68

Need to test new pull request template, once confirmed functional can close this issue, any further templates can be made in a separate issue.

Should also look into a way to configure URL used by git-fork to create pull request from client so that it uses new template.

Ended up using link in console output after pushing change. Believe this can be closed for now.
