Endnotes grouping + use of xindy #11

I also suggest to take the change in the Makefile, to allowi to add parameters to dblatex from command line.

The xindy part is already included in the latest repo. I tested the endnote change, and while it was very nice to get chapter headings in the endnote list, I believe resetting the note counter for every chapter is going to make it harder to find the correct end note. A reader coming agross note 4 will also need to look up which chapter she is reading before it is possible to find the correct note. Because of this, I believe it is better if all endnotes have unique numbers across chapters. Is that possible?
