Allow for multiple affiliations for a given author #83

Problem

@sowasser has two affiliations but there is currently not a way to accommodate this within sa4ss::get_affiliation()

Proposal

Current - just use one affiliation Future - allow for all desired affiliations to be listed

@kellijohnson-NOAA As this is just one individual at the moment, and they have two affiliations, could we merge the two affiliations into one entry so as to make it look like two but in actuality be just one entry in the authors.csv file?
