PG 13 compatibility #28

table_open/close shall be use in place of heap_open/close
Lists are now implemented as arrays hence lnext needs extra argument
include fmgr.h (probably this commit removed it from rel.h

I have confirmed your commit. And I have compared it with our temporarly fix, I am concerned about missing compatibility of old versions and so on. Therefore, for 13 support, we'd like to use our code after we completed test and fix bugs. Thank you for your support.

I added 9.6 and 10 into test matrix. They build alright. I don't believe sqlite_fdw builds against 9.5 even on Linux as it lacks UpperRelationKind.
