Add `AELOC` to `ae_ophtha` #110

Background Information

derive_var_afeye() expects AELOC - see here for an example, but ae_ophtha does not have AELOC

Definition of Done

Add variable AELOC to ae_ophtha, setting to EYE for any ocular locations and missing otherwise.
