Refactor format away from the mme-style entity-oriented model to an association oriented model
Made explicit a formalization of levels, with level 1 being basic
Modularized in terms of "profiles":
admin profile (DOB etc)
disease profile
phenotype profile
family profile (ie ped files)
environment profile (level 2)
This allows every single assertion to have evidence associated. This may be over the top for some purposes (e.g. for many purposes it's not necessary to provide evidence for a DOB assertion), but is ultimately more flexible and uniform.
