Add checkbox to consent to having patients participate in MME #52

Mike suggested a new checkbox at the top of the patient record with the other consents. The text should be something like: "Unless you set the visibility to Private, this patient will be matchable through the MatchmakerExchange and you may receive occasional match notifications."

It should be checked by default.
