This was originally reported for Graphing Lines in https://github.com/phetsims/graphing-lines/issues/40, but was demonstrated to occur for other sims with games. See the discussion in that issue for details.
This has only been reproduced (consistently) on the iPad2 belonging to @amanda-phet. Other iPad2's do not exhibit this behavior.
