(Left a comment on the forums, but thought I'd put a formal issue here as well)
I switched over to this driver for my GE Plug-in Dual Smart Switch, Independant Control (ZW4105). Before (in the DTH days) this reported as a Z-Wave Dual Switch and would create two devices so that each outlet on the Smart Switch could be controlled independently.
However, when the migration occurred, it showed up only as a "Z-Wave Outlet"... not a "Z-Wave Dual Switch" like it had before. So I switched over to these drivers to see if that would fix the problem. Unfortunately, it hasn't.
When I enroll in these drivers and set my switch to use them, it only shows a single device in the ST app and turning the power on/off will control both outlets simultaneously instead of independently.
