Index / constraint cleanup #133

Make sure all indexes / constraints are dropped
Make sure all tables are truncated
Add additional constraints for tables that are related
Comment out constraints that are violated by actual discogs data
Change index on track titles to full text, to handle long titles

Sorry, I didn't mean to submit this PR. I'm new to github, and I meant to create a PR to my own repo that I cloned from yours. I didn't realize that it defaulted to merge back upstream.
Thanks for this great project that makes it easy to load the discogs data into postgresql!
