Add QR code support #659

The PhoneGap Mobile App redesign includes the ability to scan a QR code as an alternative to using the IP address. This means that both the CLI and Desktop app need the ability to display a QR code and IP address when serving an app.

There is no deadline set at the moment, but an estimate would be end of 2016
We can reuse the QR code node library used by node-phonegap-build
