Add Darian and Steve as package maintainers. #402

This pull request adds @afshin and @blink1073 as maintainers for every PhosphorJS package, in line with the new governance model.

Also, please add my name/handle to every packages as well.

Sure! I thought that was explicit in the chief executive designation, but it is sensible to make it clear on each package. I'll update the PR.
