Move exceptions to the main namespace #6

This is so that we are consistent with other PSRs.

Since https://github.com/php-fig/fig-standards/pull/830 was merged this must be merged to reflect those changes (that's just a mirror of #830). @moufmouf or @KorvinSzanto can you merge this?
