Update Documentation #267

Hi there! I just installed this promising library, but the documentation seems to be outdated. Nor Gettext\Scanner\PhpScanner class anymore, for example. Are you planning on updating the documentation?

Sorry, Just noticed that I just hadn't required the gettext/php-scanner package. Closing.
