Add last modification info and contributor list #977

Add last commit date/time and author, as well as the list of all contributors to each documentation page.

Correct the casing and add the missing word "to" to the "Learn How To Improve This Page" link.

Needs https://github.com/php/phd/pull/115 for it to work.

Once the commit history files are generated and uploaded to the documentation repositories, the "Improve This Page" section will look like this:

And clicking the "All contributors" link would show a list of all who authored a commit to the current page like this:
