Update minimum versions of development tools

• re2c:
  • PHP 5.1 required 0.9.11+
  • From PHP 5.2 to 8.2: 0.13.4+
  • From PHP 8.3 and later 1.0.3+

This syncs the version ranges style and adds URL anchors so that sections can be linked.
