Fix obsolete references for Mantis #995

All official sources refer to Mantis like it's still there. Please remove those references, and if possible mention in each source when was Mantis removed and whether there any migration took place:

  1. CONTRIBUTING.md in GitHub itself (Update: done)
  2. The "self-host installation" link in the blog
  3. The "self-host installation" link when clicking "Show Full Post" in the forum
