Apply Transform occasional error #1

Simple test: FLIRT registration 3D->3D, save transformation.

Go to Apply Transform, select the transform obtained and apply to different 3D data:

Can't seem to reproduce this, and error is from the process so the backtrace is not helpful. Will leave open since it can clearly happen and hopefully next time get steps to reproduce.

Still not reproducible and no helpful info to go on - closing until we have a test case
