Create a template upon creating a Pull Request.

Some potential ideas:

What does this PR do? What issue/PR is this PR related to? What issue does this PR close? What tests did the user for this PR? Did the user provide screenshots/videos where necessary/relevant? Has the user read the Contributing Guidelines on issues?
