Ameritrade support

As mentioned in #26 we can get free L2 from TD Ameritrade with margin accounts.

How to get this going:

• a reddit post showing how to setup the API
• the developer getting started
• the simple app auth guide
• the data streaming tutorial which supposedly has depth of book support though it's not documented?

Fun times, so the app creation wasn't working in firefox but works just fine in chomium..lolz
