Improve slack self-invite mechanism #1

Instead of posting 30-day self invite links on meetup, I suggest adding a self service form to the website. That will help drive traffic to the website and also make it more hands off. I did something similar to one of my other projects by iframing a heroku app (details below). With my project I'm paying ~$7/mo for heroku so it removes sleeping (to load more quickly) but I ran it using the free tier for a while and it worked just fine, just a little slow for my taste.

PR it to us? I have a branch up to add a link to slack, but I didn't go as far as you are suggesting.
