Cert rotation doc update for shared.ingress_issuer #113

See https://github.com/cert-manager/cert-manager/issues/2478 for a description of the limitations of cert-manager for the rotation of CA certs. Note that this is an advanced topic that we would expect the TAP cluster issuer docs to cover, so we opted to limit the content to a warning about the risk of service downtime when manually managing a CA Issuer. If this seems like too much information to include in Application Service Adapter documentation, we're happy to remove it. The intent here was to offer an initial sketch of the closely related concern for a platform operator.

Also note that we would like to cherry-pick this documentation back to the branches for the 1.1 and 1.2 release documentation.
