Fix instructions for docker hub installation #64

When using docker hub as the container registry, image tags must use "index.docker.io" but the secret must specify credentials for "https://index.docker.io/v1/"
This commit separates the two settings in the instructions and provides the correct answer for each.

I think we will want to cherry pick this change into the 1-0 branch also.
