Team Fact Sheet is a great way to pull in contact information, status, and location of Task Forces. However, the FEMA Incident Support Team would like to leverage SharePoint Lists for a "T-Card" System.
The idea is Team Fact Sheet entries would create a "T-Card" when filled out. Ideally if points of contact or other information changes, it updates on the SharePoint List as well.
Use only the initial entry to keep the list up to date.
We may want to link back to the unique Team Fact Sheet Form to catch updates but don't want to create new cards everytime a TFS is edited.
Alternatives
We have seen people use Trello and Planner for this.

There are resources in the T-Card Board that are not Task Forces and would not be filling out the Team Fact Sheet. Therefore, there may be multiple ways to create a T-Card.
