[OMP] Add a sample CSV file for the CSV importexport tool #10117

Describe the bug The CSV importexport tool doesn't have a sample CSV file to give the user a direction to follow when using this tool. To make this job easier, I suggest creating a sample.csv file with a data example for the user.

What application are you using? OMP 3.3.0
