add support for taking annotation from a WFS DescribeFeatureType schema #167

and use if the feature editor form. This adds support for reading titles and help text from the schema.

This work depends on https://github.com/geoext/geoext/pull/63 and on previous work done in OpenLayers https://github.com/openlayers/openlayers/pull/762

@ahocevar if you have time left this week, it would be great to get a look at this PR as well as the GeoExt one (this is SFMTA work).
