This PR addresses the feedback provided in this comment: https://github.com/pln-fing-udelar/covid19-qa/pull/4#pullrequestreview-404553647

Changelog:

Add .env.template to document environment variables
Add dev proxy target url as an environement variable (it was hardcoded) I'm using a local proxy to serve both the frontend and backend on the same domain and avoid CORS issues
Track package-lock.json into git
Remove unused test utilities
