Add onboarding instructions to README.md, removed description of session token scope #6

Adds sample code for how to onboard to CosmosDB.Extensions.SessionTokens.AspNetCore. Also removes mention of session token scoping, because I'm not 100% certain that statement was true.

In addition, this change reformats the file.
