Add amendment date to header #7

Added the most recent amendment date so that it is clear that the "Bylaws" is a living document and as such has and will continue to change.

People never remember to update dates like that, and the updates clutter git history. You're better off just saying that these bylaws are a living document and include a pointer to the git repository instead.

Agreed. As long as we keep it in github, we should be able to see all changes anyway...it's when we (naively) view it in raw text that it gets confusing.
