DOCX equation conversion fails if there are equations in footnotes #45

For example, in nQCUZMn1CE. In that case, there are 136 equation placeholders but 139 equations in the Pandoc soup, suggesting that perhaps the three equations in footnote 1 are not properly found by the regex or maybe they are not handled by Mammoth.

Fixed in bec2d89 by parsing word/footnotes.xml for equation placeholders at the same time as word/document.xml.
