Aviation incident report sample added

Hi

Please find in this pull request my Aviation Incident Report Syntex document understanding sample. These are publicly available airline incident reports that I used for a POC.

@tomresing - spotted me showing a screenshot of the Regular Expression on Twitter a while back and I promised to upload it to the Syntex samples site.

I've made a few changes to the Readme file but feel free to ask me to change if necessary?

• Added Types to the Extractors table so people can see at a glance which types of extractors are in the samples i.e. Phrase, Proximity, Regular Expression?

Made the following changes to reflect other PnP Repos

• Added Sample table so users who submit are given credit for their submission - link to their GitHub
• Added PnP hidden image for tracking page view stats. If you are happy with these changes above I'm happy to create pull requests for the other samples and change them? Also would very willing to help to populate the default pull request info displayed (when a new pull request is created) so we can guide other people to submit their samples.
