(Feature) "Add claim to identity" page #207

Closes #202

Add a link to verified physical address that redirects to "Add claim to identity contract" page. Used alerts and a similar design to what can be fonud on register address page.

@phahulin @pashagonchar there is not much to show here, and I used already-existing UI elements of the current version of PoPA. Is this ok? What about the wording (we can add/change anything you believe is relevant)?
