  1. Provide error message
  2. Provide crashdump (if any)
  3. State any changes made to the plugin that wasn't pushed to GitHub
  4. Provide plugin version or commit
  5. State version of PM and if using a spoon
  6. Provide screenshots (if applicable)

If you haven't already noticed, most orphans are spammed with issues which are either spam or don't provide enough information.
