UI for loading documents #39

 We need to adjust the loading UI on (at least) the following two issues:
        1. currently lack of index creation function 
        2. musicdoc upload not finished
 And should we keep a dropdown menu for formats or just get the format info from filename?

still a lot to improve, does not work currently
