Add problem icons to example pages such as

https://polyrhythm.humdrum.org/example/?id=310

Currently displayed as a red "P":

Implemented with commit https://github.com/polyrhythm-project/polyrhythm-website/commit/da3228e22400a32322a686fcf23748a0d62f6d09

Now renders as red triangle:

Ideally mouse-over on triangle will display a tooltip listing the problem.
