Issues for R686x #111

  1. Added measure numbers
  2. Added slurs throughout LH Pno;
  3. As previously discussed, grace note slurs do not show on VHV, they are expressed as ties currently: fixed by replacing with slurs - some formatting issues.
  4. Cross-staff triplet in mm. 3 was hidden, added notes and cross-staff command. Number is above text for some reason.
  5. Tenuto + accent colliding in RH mm. 2
  6. 8va's are missing their close
