Put in extra message when starting up an app #75

Currently we upload a tar file to the UserAppServer and tell the user their
app has been uploaded. We then wait for the load balancer to perform a
redirect to the app, but this may take up to five seconds and no indication
is given to the user that this will take a while.

An extra message has been added to run-instances and upload-app indicating
this as needed.
