Screen C.3.4 update the top form

Currently this form contains a few links, one to select "Oorsponkelijke 
schuldeiser" and one "Deze schuld is uitbesteed aan een deurwaarder". 

Clicking these links will show a popup screen.

These links should be changed into text input fields.

When the user clicks on the text input field, the same popup as previously 
should be displayed. When an option is selected, the name of the selected 
creditor should be displayed in the text input field (and its key value should 
be placed in the hidden input field as before).

Also reorder the fields in this form:
1. Datum brief
2. Deze schuld is uitbesteed aan een deurwaarder
3. Oorspronkelijke schuldeiser

For the rest of the fields use the order as the fields are currently displayed.
